Qualifications

• Bachelor's in social work, psychology or related clinical field required.
• At least 2 years of experience with individuals with serious mental illness.
• Working knowledge of Philadelphia's BH and criminal justice systems.
• Experience with navigation and linkage to treatment and supports.

Responsibilities

• Provide navigation services to connect participants to behavioral health, housing, and benefits.
• Assist CP Mental Health Court participants and manage caseloads.
• Collaborate to develop policies for Resource Hub and court programs.
• Triage referrals to BHJD programs and supervise Navigators.
• Attend mental health court hearings and supply treatment letters as needed.
• Document work and maintain confidential records in EHR.
