Job summary

Bridges of Colorado seeks a Service Professional to engage participants through person-centered, solution-focused case management. • Bachelor’s degree in social justice, social work, sociology, criminology, or related field.
• Minimum 3+ years in criminal justice, behavioral health, or related field.
• Demonstrated alignment with Bridges values and equity principles.

Responsibilities

• Provide ongoing, personalized engagement with participants through case management.
• Conduct intakes, screenings, and assessments for mental health and social needs, in jail, court, home or community settings.
• Link participants to services such as case management, behavioral health and housing supports.
• Document activity and outcomes in Bridges case management system and other relevant systems.
