Job summary

Cornerstone Care Community Health Centers in Greensboro, PA is seeking an Integrated Care Coordinator to join our team. The ideal candidate holds a Bachelor's degree in Psychology or Social Work, has knowledge of behavioral health, and experience in outpatient settings.

Qualifications

• Bachelor's degree in Psychology or Social Work; Behavioral Health experience.
• Knowledge in behavioral health and basic medical knowledge.
• Outpatient clinic experience preferred but not required.

Responsibilities

• Detect at-risk patients for mental health and substance abuse screening.
• Screen primary care/psychiatry patients for mental health disorders.
• Coordinate referrals to outpatient and inpatient treatment.
• Provide psychoeducation and support for patients and families.
• Assist in quality improvement and grants reporting.
