Job summary

Bellin Health in Green Bay is seeking a Behavioral Health Intake Specialist - Intake/Triage to perform case review, telephone assessments, and triage for appropriate level of care.

The role supports a multidisciplinary team, coordinates referrals, and communicates patient outcomes across the health system; requires a Bachelor's in Social Work, Psychology, Counseling, or related field and prior mental health triage experience.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Counseling, or a related field required.
• Previous experience in mental health triaging required.

Responsibilities

• Conduct case review and telephone assessments to determine appropriate level of care and safety planning when needed.
• Answer phone calls, perform telephone assessment and triage, and schedule full patient assessments with appropriate resources.
• Collaborate as a member of the multidisciplinary treatment team and coordinate communications across the health system.

• Hours: 32hours/week
• FTE Status: 0.80 FTE, Full-Time
• Work Schedule: P.m. shifts, with an every other weekend rotation; P.m. shifts - 3:00pm - 11:30pm
