Qualifications

• Bachelor’s degree in social work or behavioral health from an accredited program is required.
• Experience in behavioral health intake or admission coordination is not required but beneficial.

Responsibilities

• Assess referrals for appropriate levels of care and determine admission or other disposition.
• Coordinate with referral sources and clinical teams to gather information for care decisions.
• Documentation and updates to care coordination systems.
• Clinically assess referral data to determine inpatient criteria and arrange appropriate care paths.
• Connect patients and families with community resources and supports when indicated.
• Collaborate with hospital staff and protect services as needed.
• Demonstrate empathy, communication, organization, critical thinking, and patience in daily work.
• Support evolving scheduling needs, including on-call duties as required.
