Responsibilities

• Respond to inquiry calls and complete inquiry forms accurately.
• Schedule assessments and document follow-up on pending cases.
• Conduct assessments at facility or other locations and document triage.
• Coordinate pre-certification and admissions processes for program entrants.
• Provide respectful patient services to promote dignity and satisfaction.
