Community Health Programs (CHP) is seeking a part-time Behavioral Health Intake Coordinator for a hybrid role at the Neighborhood Health Center in Pittsfield, MA. Qualifications

• Bachelor's degree in Psychology, Social Work, Human Services, Public Health, Counseling, or a related field.
• Minimum of two years of experience in healthcare, behavioral health, care coordination, case management, or community-based services.
• Knowledge of behavioral health conditions, substance use disorders, and community resources.
• Strong communication, organizational, triage, and customer service skills.
• Experience working with diverse and underserved populations.
• Proficiency with electronic health records and Microsoft Office applications.

Responsibilities

• Behavioral Health Intake & Assessment: Manage the behavioral health referral and intake queue for all CHP sites.
• Care Coordination & Population Health: Identify high-risk patients, collaborate with Care Coordinators, Community Health Workers, and Primary Care teams to address barriers.
• Referral Management & Provider Placement: Match patients with the most appropriate behavioral health clinician and coordinate documentation.
• Resource Management: Maintain a comprehensive directory of behavioral health resources and educate patients about services.
• Quality Improvement & Health Equity: Monitor referral trends, wait times, and outcomes; promote equitable access to services.
• Documentation & Compliance: Maintain timely EHR documentation and ensure HIPAA compliance.
