Qualifications

• Three years of experience providing coordinated supportive services/care coordination.
• Experience working in the behavioral health field, preferably with multi-system youth.
• Advanced communication skills both oral and written.

Responsibilities

• Supervise and coordinate activities of Referral Specialists.
• Oversee care coordination intake and referral process.
• Analyze management reports and workforce metrics.
