Qualifications

• A high school degree or GED equivalent is required.
• A bachelor’s degree in healthcare, behavioral health or related field is preferred.
• Three years of experience providing coordinated supportive services/care coordination services is required.
• Previous supervisory experience is preferred.

Responsibilities

• Supervise and coordinate the activities of Referral Specialists.
• Oversee the OhioRISE care coordination intake and referral process.
• Analyze management reports and workforce metrics.
• Monitor OhioRISE-specific onboarding activities.
• Maintain oversight of training documentation and reporting requirements.
