Position Summary: Provides professional behavioral counseling services to designated patient population, discharge planning, and related family support.

Qualifications

• Bachelor’s degree in social work or related field as stated.
• Master’s degree in behavioral health is acceptable.
• Experience or licensure not required.

Responsibilities

• Assists patients and families with physical, emotional, and social concerns during hospital stay.
• Provides discharge planning services to patients on assigned units.
• Completes documentation prerequisites for placement/authorizations prior to discharge.
• Conducts initial assessments and ongoing documentation for background/social data.
• Connects patients/families with community resources to aid treatment and recovery.
• Collaborates with staff and physicians and communicates progress via chart notes.
• Demonstrates empathy, communication, organization, critical thinking, active listening, and patience.
• Adheres to policies, completes education, and participates in quality improvement initiatives.
