Qualifications

• Experience in healthcare, behavioral health, care coordination or related patient-facing work.
• Ability to manage a high-volume caseload across multiple facilities.
• Proficiency with electronic health records (EHR) and digital tools.
• Strong communication, organization and cultural competency.

Responsibilities

• Manage a caseload of patients in the behavioral health program with monthly screenings.
• Elevate changes in condition to the BHCM or clinical team as needed.
• Coordinate with facility leadership, nursing staff, therapy teams, social services and IDT members.
• Complete required monthly documentation per PHA policies and maintain EHR records.
• Travel between facilities within the regional territory on a rotating schedule.
