Job summary

Responsibilities

• Coordinate additional behavioral health services as needed.
• Serve as on-site contact for assigned facilities.
• Communicate with facility leadership and care teams.
• Assist scheduling and on-site logistics for services.
• Complete monthly documentation under policies.
• Maintain patient records in the EHR and ensure privacy.
• Collaborate with BHCM on caseload updates and workflows.
• Follow escalation pathways for safety concerns.
• Travel between facilities within the regional territory.
