Job summary

Infinity Health is seeking a Behavioral Health Facilitator (CNA/MA/LPN) for a PRN role based in Centerville, Iowa, with travel to other Infinity Health locations as needed.

In this role you will welcome patients, prepare them for visits, support TeleMed sessions, check vitals, manage chart updates, schedule appointments, and assist with medications and follow-ups, all while delivering compassionate, quality care in a flexible team environment.

Qualifications

• Must have MA or LPN credentials as listed.
• Strong communication skills and attention to detail.
• Comfort with technology and willingness to support patients and teammates.

Responsibilities

• Welcome, room, and prepare patients for appointments
• Facilitate TeleMed behavioral health visits
• Obtain vital signs, reconcile medications, and update patient charts
• Schedule in-person and virtual appointments
• Administer injections as permitted by license or certification
• Respond to patient calls and relay information to providers
• Complete patient follow-up and engagement activities
• Perform drug urine screenings and coordinate outside lab orders
• Complete medication prior authorizations
• Travel to other Infinity Health locations to provide coverage as needed

We're looking for a compassionate and dependable CNA, Medical Assistant, or LPN to join our Behavioral Health team in a PRN (as-needed) position based primarily in Centerville. This role will also provide coverage at other Infinity Health locations as needed.

What You'll Do

As a Behavioral Health Facilitator, you'll work closely with our ARNPs, Psychiatrists, and care teams to create a positive and coordinated experience for our patients. You'll:

• Welcome, room, and prepare patients for appointments
• Facilitate TeleMed behavioral health visits
• Obtain vital signs, reconcile medications, and update patient charts
• Schedule in-person and virtual appointments
• Administer injections as permitted by license or certification

Who We're Looking For

We're looking for someone who is compassionate, dependable, organized, and comfortable working with patients receiving behavioral health services.

Candidates must have one of the following:

• Medical Assistant (MA)
• Licensed Practical Nurse (LPN)

You'll also need strong communication skills, attention to detail, comfort with technology, and a willingness to support patients and teammates wherever you're needed.

Travel: Coverage at other Infinity Health locations may be needed
