Job summary

Qualifications

• High School Diploma or equivalent required, six months related field experience or 12 college credits in human services preferred.
• Must be at least 18 years old and able to pass pre-employment screening.
• Valid driver's license and reliable transportation may be required.

Responsibilities

• Assist individuals in identifying and communicating needs for services and support.
• Promote healthy living and implement wellness strategies.
• Encourage self-advocacy and informed personal choice.
• Provide transportation to appointments and community activities.
• Support independent living skills and daily living tasks.
• Document all services to ensure accurate records and communication.
• Administer medications where state regulations allow and maintain confidentiality.
