Job summary

Westpark Springs in Richmond, TX is seeking a Discharge Coordinator (PRN) to support continued patient recovery after discharge. You will coordinate aftercare plans, assess patient needs, and collaborate with the care team to arrange follow-up services and resources.

The role emphasizes documentation, interdepartmental communication, crisis support, and adherence to HIPAA and regulatory standards to ensure a smooth transition of care for patients across inpatient and outpatient settings.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or relevant state licensure.
• 3+ years clinical inpatient behavioral health experience.
• Unencumbered clinical license if eligible; CPR and Handle with Care within 30 days.

Responsibilities

• Provides aftercare coordination for patients who need ongoing medical/psychosocial care following discharge.
• Creates and maintains up to date aftercare coordination community resources.
• Displays active involvement in treatment planning by setting up aftercare appointments.
• Provides resource information to patients and families for future care needs.
• Communicates with clients, families, aftercare providers, and referral sources.
• Creates a detailed Discharge Summary and Contract for each patient.
• Works with interdisciplinary team including physicians, utilization review and nursing staff.
• Completes all hospital and client documentation thoroughly and timely.
• Demonstrates crisis intervention and de-escalation skills when needed.
• Understands confidentiality and HIPAA regulations.
• Demonstrates knowledge of clinical regulations (TJC, Medicaid, state/federal laws).

Education

Bachelor's degree in social work
