Job summary

DESC's Crisis Solutions Center offers stabilization services to clients in behavioral health crises. Case managers work with a multidisciplinary team to create client-centered care and safety plans, and ensure integration with mental health, substance use, and primary care teams.

Case managers develop care plans, help clients access community resources, and maintain comprehensive records. The role includes teamwork, trainings, and a commitment to a safe, therapeutic environment.

Qualifications

• Bachelor's degree in social work, psychology, or related field, or 1 year relevant paid work experience.
• Ability to meet Washington DOH RAAC requirements or superseding credential.
• Valid Washington driver's license and insurable driving record.
• Ability to obtain a Washington Food Handler's Permit.
• Interest in engaging with difficult clients in mental health/substance use programs.
• Pass WA State background check.

Responsibilities

• Participate in psychiatric consultation, supervision, and team meetings.
• Create and implement short-term care plans and discharge dispositions.
• Facilitate access to community resources and coordinate with care teams.
• Perform verbal de-escalation and maintain a safe, therapeutic environment.
• Maintain clinical and administrative records including goal planners and progress notes.
• Provide phone consultation and perform administrative duties.
• Assist with meals, laundry, cleaning, and biohazard cleanup as needed.
• Provide transportation for clients when needed.
• Attend onboarding and ongoing trainings.
