Job summary

Access Community Health Centers in Madison, WI seeks a Behavioral Health Consultant to join the primary care team, providing patient care, triage, and psychoeducation within a collaborative setting.

You will develop behavioral change plans, train staff on behavioral health protocols, and address medical and behavioral health issues with evidence-based approaches. A doctoral or master’s degree and Wisconsin licensure are required; bilingual English/Spanish is a plus.

Qualifications

• Doctoral or master’s degree as listed in qualifications.
• Wisconsin licensure as a clinical professional.
• Experience in medical settings preferred.

Responsibilities

• Provide patient care and triage within primary care.
• Deliver psychoeducation and patient education strategies.
• Develop behavioral change plans for patients and protocols for target populations.
