Job summary

Carle Health is seeking a Licensed Behavioral Health Practitioner to provide assessment, intervention, and consultation for individuals and groups with behavioral health needs. This role includes crisis evaluation, treatment planning, and collaboration with community and care teams.

The LBHP holds licenses under IDFPR, with required master’s degrees in social work, psychology, counseling, or related fields and at least two years of behavioral health experience.

Qualifications

• Must hold LCPC, LCSW, LMFT, or Licensed Clinical Psychologist credentials under IDFPR.
• Master’s degree in social work, psychology, counseling, or related field.
• Minimum 2 years of behavioral health experience.

Responsibilities

• Performs diagnostic assessments and determines treatment needs with patients.
• Creates treatment plans with goals aligned to assessments.
• Provides behavioral health therapy to individuals, families, and groups.
• Coordinates with Emergency Response Service for high-risk clients.
• Maintains records per regulatory and organizational policies and standards.
• Acts as liaison with community agencies, primary care, court, and managed-care entities.
