Job summary

Carle Health is seeking a licensed behavioral health professional to provide assessment, intervention, and consultation within a community-based setting. The role includes crisis evaluation, therapy for individuals and families, and coordination of treatment plans with providers and community agencies.

The LBHP operates under supervision as a Licensed Practitioner of the Healing Arts in accordance with Medicaid Community-Based Mental Health Services rules.

Qualifications

• Licensed professional with one of LCPC, LCSW, LMFT or Licensed Clinical Psychologist licenses as recognized by IDFPR.
• Experience providing behavioral health assessment, intervention and crisis evaluation.
• Ability to coordinate treatment plans and communicate with care teams and agencies.

Responsibilities

• Performs diagnostic assessments and determines immediate treatment needs with patients.
• Develops and implements treatment plans with patient input and measurable goals.
• Provides behavioral health therapy to individuals, families and groups.
• Coordinates with emergency/crisis services when safety concerns arise.
• Maintains documentation in accordance with policies and regulatory requirements.
