Baptist Health is seeking a licensed clinical social worker or equivalent to coordinate and assess patients referred to intensive outpatient programming within our Behavioral Health services. The role involves collaboration with psychiatrists, patients, families, and insurance verification to determine level of care.

Responsibilities include coordinating treatment plans, facilitating group therapy and psychoeducational classes, providing brief counseling, and assisting with FMLA/short-term

Qualifications

• Master's degree required in a related field.
• Active Kentucky License: LCSW, LPCC, or LMFT.
• 2 years of job-related experience required.
• Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred.

Responsabilités

• Participates in the formulation of a treatment plan, implementation and evaluation.
• Implement therapeutic, evidenced based group therapy and psychoeducational classes to meet the needs of the patients.
• Provide brief therapeutic counseling.
• Assist patients with paperwork needed for FMLA and Short-term Disability in order to be away from work while in treatment.
• Develops and maintains professional relationships with treatment team members, hospital and medical staff and community partners.

• Start Time of shift - varies between 8a - 2p
• End Time of shift - varies between 1p - 8p

Job Summary

• Coordinates the approval and level of care determination in cooperation with the psychiatrist,patient/family and insurance verification.
• Coordinates and facilitates outside referrals and placement if needed
