PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW/LCSW) to join the Behavioral Health team in Mercer, PA. This full-time role is based in the Mercer County Prison, with Monday–Friday, 8:00am–4:30pm hours.

The position requires clinical assessments, risk evaluations, and coordination with external forensic services, while maintaining NCCHC standards. Competitive compensation and benefits apply.

Competenze

• Must be licensed as LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Master’s degree or greater in Social Work, Psychology, or related Human Service field with clinical coursework.

Mansioni

• Conduct clinical assessments based on referrals; document findings including mental status exam, case conceptualization, diagnosis, and treatment plan; refer to psychiatry when appropriate.
• Perform risk assessments using structured interviews and tools; manage inmates at risk for suicide or self-harm per policies.
• Serve as PrimeCare Medical mental health liaison for services inside/outside facility.
• Facilitate admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitor all mental health services and ensure standards align with NCCHC and psychiatric/psychological norms.
• Participate in Quality Assurance Committee.
• Develop and conduct in-service education programs per annual calendar.
• Collect data and prepare monthly performance reports.
