Job summary

PrimeCare Medical is seeking a Behavioral Health Professional in Mercer County Prison, Mercer, PA. This full‑time role runs Monday–Friday, 8:00am–4:30pm, with responsibilities including clinical assessments, risk assessments, and functioning as a liaison for internal and external mental health services.

Required licensure in PA (LPC/LSW/LCSW) and a master’s degree in Social Work or Psychology, with a minimum of two years’ experience.

Qualifications

• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with clinical coursework.
• Must be licensed as a LPC, LSW, or LCSW in the Commonwealth of Pennsylvania.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments based on referrals and self-referrals with documentation and treatment planning.
• Perform risk assessments using structured interviews and tools; manage at‑risk inmates per policy.
• Serve as PCM mental health liaison for services outside the facility.
• Facilitate admission/placement of inmates to forensic hospitals when necessary.
• Monitor all mental health services to ensure quality of care and standards compliance.
• Contribute to Quality Assurance through committees and annual in‑service education.
• Collect data and prepare monthly performance reports.
