Job summary

PrimeCare Medical is seeking a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) to serve as a Behavioral Health Professional traveling to multiple county jails in Pennsylvania, including Greensburg, Somerset, Washington and Ebensburg.

The full-time position requires travel, with a standard 40-hour work week (Mon-Fri, 8:00am–4:30pm) and duties that include mental health assessments, risk assessments, treatment planning, and coordination with external psychiatry

Qualifications

• Licensed as LPC or LCSW in Pennsylvania.
• Master’s degree or higher in Social Work, Psychology, or related field.
• Minimum of two years of experience.

Responsibilities

• Conduct clinical assessments and document findings including mental status exam and treatment plan.
• Perform risk assessments and manage at-risk inmates per policy.
• Serve as PCM mental health liaison for external services.
• Facilitate admission to forensic hospitals when needed.
• Monitor mental health services and ensure standards of care.
• Participate in Quality Assurance and develop in-service training.
• Collect data and prepare monthly performance reports.

Job description

PrimeCare Medical is currently looking for a Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) to work as a Behavioral Health Professional traveling within the following facilities:

• Westmoreland County Jail: 3000 S Grande Blvd, Greensburg, PA 15601
• Somerset County Jail: 127 East Fairview Street, Somerset, PA 15501
• Washington County Jail: 100 W. Cherry Street, Washington, PA 15301
• Cambria County Jail: 425 Manor Drive Box 595, Ebensburg, PA 15931

This position requires ability to travel and work in each of the facilities listed above. This a Full-Time 40-hour position, Monday - Friday, during normal business hours. (8:00am- 4:30pm).

Job Responsibilities

• Conduct clinical assessments based on referrals and self-referrals. Assessments include documentation of findings, including at a minimum a mental status examination and case conceptualization, diagnosis, and treatment plan. Makes referrals to psychiatry when appropriate.
• Risk assessments are to be conducted utilizing structured interviews and risk assessment tools. Inmates at risk for suicide and / or engaging in self-harm are managed utilizing existing protocols according to current policy and procedures.
• Serve as the PrimeCare Medical (PCM) mental health liaison for mental health services, outside the facility.
• Facilitates admission and placement of inmates/patients to forensic hospitals when necessary.
• Monitors all mental health services provided.
• Monitors clinical services to ensure that standards of care are in accordance with NCCHC, and accepted psychiatric/ psychological standards.
• Is a member of the Quality Assurance Committee.
• Develops and conducts in-service education programs according to annual in-service calendar.
• Collects data and prepares monthly reports, which reflect performance.

• Salaries are negotiable and commensurate based on experience. Annual Salary range is $65,000.00 - $75,000.00

Requirements

• Must be licensed as a LPC or LCSW in the Commonwealth of Pennsylvania.
• Must hold a Master’s degree or greater in Social Work, Psychology, or related Human Service field with a concentration in clinical course work, training and/or experience.
• Minimum of two years of experience.
