Job summary

Baystate Health in Holyoke, MA is seeking a per diem Behavioral Health Clinician to join the Child Partial Hospitalization Program. Per diem hours are Monday-Friday 8:00am-4:30pm with no weekends or holidays.

Minimum qualifications include a Master’s in Social Work or Mental Health Counseling and licensure eligibility (LICSW/LMHC). Experience with children is preferred; duties include psychosocial evaluations, treatment planning, group facilitation, and discharge planning.

Qualifications

• Masters in Social Work or Mental Health Counseling (required).
• LICSW, LICSW or LMHC licensure (required).
• 1 year experience working with children (preferred).

Responsibilities

• Complete psychosocial evaluations including presenting symptoms, past history, assessment of mental status and risk assessment through patient interviews, collateral contacts, and record review.
• Identifies the reason(s) for admissions and in collaboration with the patient develops treatment goals and weekly updates.
• Facilitates/co-facilitates psychoeducational and psychotherapeutic groups to address treatment objectives.
• Complete discharge plans which address aftercare recommendations, step-down options, community supports, or other treatment needs.
• In collaboration with psychiatric providers, completes discharge summaries.
• Actively participates in the Multidisciplinary Treatment Team Meetings to coordinate the highest quality of care to patients.
