Zusammenfassung

Easterseals Blake Foundation is seeking a Behavioral Health professional to coordinate services for adults and children enrolled in public behavioral health programs in Arizona. You will perform intakes, assessments, and provide therapeutic and support services, while coordinating with providers and community agencies.

The role requires strong clinical knowledge, ability to manage high volumes, and capability to handle crisis situations.

Qualifikationen

• High School Diploma/GED with 1 year behavioral health or related work/volunteer experience or an Associate’s/Bachelor’s degree in Behavioral Health required.
• Bilingual (Spanish) preferred.
• Regulatory requirements including background checks and fingerprinting.

Aufgaben

• Assess member behavioral health needs and eligibility; develop and monitor service plans.
• Provide intakes, assessments, annual reviews, and therapeutic/support services.
• Coordinate communication with providers and community agencies for care.
• Provide rehabilitation and living skills training; supported employment services.
• Offer case management, family support, peer support, and transportation services.
• Conduct rapid responses/intakes as needed at assigned locations.
• Perform crisis intervention and maintain productivity and quality standards.
• Document and report member services per agency/funding rules.
• Engage in ongoing professional development and cross-team communication.
• Perform other duties as assigned.
