Job summary
Community Bridges, Inc. (CBI) seeks a Behavioral Health Clinician to join our interdisciplinary treatment team across Crisis, Inpatient and Transition Point programs in Arizona.
You will coordinate with PCTs, RNs, and BHMPs to ensure a safe environment and high-quality care for individuals in crisis, trauma, or behavioral health cases. You will complete behavioral health assessments and service plans, document services, and coordinate discharges while engaging families and caregivers to support
Qualifications
- Bachelor’s degree in a field related behavioral health is required.
- In lieu of a bachelor’s degree, a high school diploma or GED plus one year of health care related experience will be considered.
Responsibilities
- Coordinate with Patient Care Technicians (PCT), Registered Nurses (RN), and Behavioral Health Medical Providers (BHMP) to ensure a safe environment and quality care.
- Complete behavioral health assessments and service plans for admitted individuals.
- Document provided services and coordinate discharges from CBI programs.
- Coordinate care with the family or other significant others.

CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models.
The Behavioral Health Clinician (BHC) is a member of the interdisciplinary treatment team in CBI Crisis, Inpatient and Transition Point programs. The Behavioral Health Clinician coordinates with the Patient Care Technicians (PCT), Registered Nurses (RN), and Behavioral Health Medical Providers (BHMP) to ensure a safe environment and that quality care is provided to all presenting for services. The Behavioral Health Clinician completes the behavioral health assessment and service plan for all individuals admitted to their assigned program. The BHC documents provided services and coordinates discharges from CBI programs. The BHC coordinates care with the family or other significant others.
- Bachelor’s degree in a field related behavioral health required. In lieu of a bachelor’s degree, a high school diploma or GED and a minimum of one (1) year of full-time health care related work experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations will be considered.
