Exc1005Exceh in Kentucky seeks a qualified clinician to conduct comprehensive patient assessments and psychosocial histories, and to document activities within required timeframes. The role collaborates with the treatment team, manages authorizations, and supports discharge planning while educating patients and families.

The position requires at least a Master’s degree in a relevant field and clinical experience, with strong communication and familiarity with Payor/Insurance benefits.

Qualifications

• Master’s degree or higher in Social Work, Psychology, Rehabilitation, Activity Therapies, Counseling, Education or related fields.
• One year or more of experience in a clinical setting.
• Knowledge of payor/insurance benefits and basic office technology.

Responsabilités

• Completes patient assessments and psychosocial history at initial presentation.
• Documents clinical activities within required time frames per standards.
• Coordinates with treatment teams; refers to appropriate facilities as needed.
• Obtains authorizations/certifications for treatment and communicates with stakeholders.
• Provides patient/family education and participates in educational activities.
• Attends treatment team meetings and supports discharge planning.
• Provides and documents crisis intervention as needed.
