Job summary

Crossroads Community, Inc. seeks a motivated Client Support Specialist to join the Psychiatric Rehabilitation team.

You will assess client needs, develop rehabilitation plans, document progress, and coordinate services for individuals with behavioral health needs on-site at the program in Centreville, MD. The role emphasizes teaching skills, resiliency, and independent living, with transportation support and collaboration with families and other providers.

Qualifications

• High School Diploma or equivalent.
• AA or BS in Behavioral Health, Psychology, Social Work, or related field and 2 years of positively referenced work history in human services.

Responsibilities

• Assist clients in assessing strengths, needs, resources and desires and in developing their Individual Rehabilitation Plan (IRP).
• Coordinate services provided by Crossroads for assigned clients.
• Assist clients in meeting their goals and assess and document their progress each month.
• Assist clients to develop resources needed to promote recovery and resiliency and life skills necessary to maintain independent living and success in life.
• Maintain clients’ medical records in accordance with regulatory and accreditation requirements.
• Utilize computerized databases or other procedures to submit accurate, timely documentation of services.
