The Behavioral Health Caseworker is responsible for coordinating services and addressing social determinants of health for Members with behavioral health conditions. Under the direction of the Population Health Manager and Supervisor, the Behavioral Health Caseworker ensures the provision of services in the least restrictive setting and transitions support across and between care settings; connects members to services that promote community living and helps to overcome any barriers to health; establishes points of contact to collaborate with community, medical, and behavioral health teams.

The Behavioral Health Caseworker meets with assigned Members in their homes or community settings. They will develop an understanding of each Member’s circumstances and needs, assess available services, and create a person-centered plan to address behavioral health needs. For physical health needs, the caseworker will consult with a registered nurse care manager.

The Behavioral Health Caseworker will:

  • Support Members during behavioral health transitions through assessment and coordination of resources, providing referrals and linkage to community resources, and evaluating the person-centered plan.
  • Review medication lists with members and coordinate pharmacy needs to support medication adherence.
  • Monitor and update the person-centered plan through follow-up contacts, preferably in the Member’s home, based on progress, needs, and preferences.
  • Establish points of contact to collaborate with community, medical, and behavioral health teams.
  • Maintain accurate documentation of Member interactions in electronic care management platforms.
  • Develop knowledge of care management platforms, programs, policies, procedures, Member benefits, community resources, and regulatory requirements.
  • Support a positive, inclusive workplace environment, collaborating and sharing knowledge to serve a diverse Member population.
  • Conduct face-to-face visits at Member residences, provider offices, hospitals, or community locations for education and assessment.
  • Perform other duties as assigned.

Education/Experience:

  • Bachelor’s Degree in social work, psychology, sociology, early childhood education, child development, criminal justice, or related field, or in another field with relevant experience.
  • Required Bachelor's in Human Services with 45 hours of Behavioral Health training OR Master’s Degree in social work, nursing, rehabilitation, psychology, or related health/human services, with documented experience with the Member population.
  • Refer to State Requirement Document and Market Contract for additional requirements.
  • At least 2 years of professional experience working directly with individuals affected by severe mental illness (SMI), including conditions like Psychotic Disorders, Bipolar Disorders, Major Depression.
  • 2 years of social services experience engaging with clients, assessing needs, connecting to services, and monitoring progress.
  • Proficiency in MS Office, internet applications, electronic medical records, and care management platforms (e.g., Jiva).
  • Strong documentation skills emphasizing accuracy and compliance.
  • Experience assessing members, developing action plans, and teaching self-management skills.
  • Willingness to travel to members’ homes and community locations, possessing reliable transportation, a valid driver’s license, and auto insurance.
