Job summary

The Hartford is seeking a Behavioral Health Case Manager to conduct comprehensive evaluations of disability claimants and guide complex claims toward favorable outcomes. You will collaborate with multidisciplinary teams to assess return-to-work potential and functional capacity, while leveraging resources such as vocational rehabilitation and medical review experts.

Qualifications

• Master’s Degree in Behavioral Health required.
• Professional licenses such as LMHC, LCSW, LMFT, LPC or similar required.
• License to practice independently required.
• Minimum 3 years of clinical practice after degree.
• Preference for case management and discharge experience.
• Preference for managed care/insurance experience.
• Preference for crisis intervention skills.
• Strong skills in Microsoft Word, Excel, and PowerPoint.

Responsibilities

• Conducts comprehensive evaluation of a disability claimant functionality.
• Advises on highly complex claims at multidisciplinary clinical review rounds.
• Determines when claims contain quality of care issues and escalates as needed.
• Conferences with treating providers to evaluate clinical symptoms.
• Utilizes vocational rehab, risk management, physician reviews, home assessments, etc.
• Manages risk and resources on complex behavioral health claims.
• Identifies return-to-work options and barriers with internal teams.
• Reviews clinical integration systems to determine referrals.
