Qualifications

• Active and unrestricted RN license issued by a state participating in the Nurse Licensure Compact (NLC) or licensed psychologist (doctoral level), LMHC or LICSW.
• Three to five years’ experience in a medical/clinical environment or managed health plan.
• Must obtain CCM certification within three years of employment.

Responsibilities

• Assist members in navigating the behavioral healthcare system and coordinate services to improve health and well-being.
• Conduct telephonic or face-to-face health assessments to identify high-risk members and coordinate action plans.
