Job summary

CarePlus NJ Inc is seeking a Case Manager to join our Behavioral Health Home in Paramus, NJ. This on-site, part-time role (20 hours) focuses on coordinating care for adults with serious mental illness and chronic medical conditions to improve overall health outcomes.

You will lead psycho-educational groups, provide on-site and outreach services, and collaborate with a multi-disciplinary team to develop and implement comprehensive care plans using evidence-based approaches.

Qualifications

Bachelor's degree in a related field.
Two years of experience in health care or behavioral health.
Valid driver's license and must be insurable by the agency's automobile insurance.

Responsibilities

Facilitate and coordinate psycho-educational services for consumers and families on wellness topics.
Provide on-site and outreach case management, including transportation.
Ensure community supports are in place to assist the consumer.
Document observations and information in the service record and share with team.
Assist in intake, ongoing recovery, and discharge assessments and evaluations.
Collaborate to develop bio-psycho-social assessments and care plans.
Implement evidence-based interventions (e.g., CBT, MI) to help achieve treatment goals.
Participate in quality assurance activities and cross-functional teams.
