Job summary

Northern Light Acadia Hospital in Portland, ME is seeking a Full Time Behavioral Health Case Manager to join a multi-disciplinary team. You will provide comprehensive case management services to adults with serious mental illness and support children with emotional disturbances, in collaboration with primary care providers.

Work primarily in Mercy Medical Office Building with a hybrid schedule, 8:00 AM to 4:30 PM, and a pay range of $27.00–$30.90 per hour.

Qualifications

• Mental health professionals with MHRT/C provisional or full certification or eligibility.
• Valid driver's license and access to a vehicle.
• Bachelor's degree in a related field is required.

Responsibilities

• Coordinate access to mental health and substance use treatment.
• Meet clients via Zoom or in-home/community visits.
• Build therapeutic rapport and adapt communication to client needs.
• Coordinate and facilitate team meetings and care planning.
• Document in electronic medical record per policy and review treatment plans.
• Participate in monthly supervision and case consultation.
• Maintain safe environments and adhere to policies.
