Job summary

Renaissance Recovery in Fountain Valley, CA is seeking a compassionate Behavioral Health Case Manager to support clients through their treatment journey. You will coordinate care with families, clinical staff, healthcare providers, and community resources, maintaining a caseload and documenting progress in the EMR.

Qualifications

• Minimum 1 year of experience in behavioral health, substance use treatment, addiction treatment, mental health, or related setting.
• Previous case management experience strongly preferred
• Knowledge of addiction, recovery principles, treatment planning, and community resources
• CPR and First Aid certification, or willingness to obtain upon hire
• Strong written and verbal communication skills
• Strong documentation and organizational skills
• Ability to manage multiple clients, deadlines, and priorities
• Ability to maintain professional boundaries and client confidentiality
• Comfortable working collaboratively as part of a multidisciplinary treatment team
• Dependable, self-directed, adaptable, and able to work effectively in a fast-paced treatment environment

Responsibilities

• Manage a caseload of approximately 18 clients
• Complete admission notes, case management documentation, and client records on time
• Meet regularly with clients to assess needs and progress
• Provide updates to families and clinical team
• Coordinate care with outside providers and community resources
• Assist clients with insurance access and benefits
• Facilitate case management and recovery-focused groups
• Document client interactions, progress, referrals, and services in EMR
• Monitor changes in clients' mental health and substance use
• Advocate for clients and address barriers to treatment
