Job summary

Anew Behavioral Health is seeking a Behavioral Health Case Manager to coordinate care for clients within Huntington Schools in Ohio. You will assess needs, develop service plans, and collaborate with multidisciplinary teams to support recovery and independence.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, and client-centered.
• Manage case assignments, draft service plans, and review case progress regularly.
• Advocate for clients, assisting them in making informed decisions regarding their clinical status and treatment options.
• Collaborate with therapists, clients, and family members to ensure all client behavioral health needs are met.
• Facilitate clients in achieving personal independence and develop daily living skills.
• Perform other duties as assigned.
• Upholds Documentation and Billing practices associated with CARF, CMS, and training indications.
