Job summary

Granite Hills Hospital is seeking a Case Manager to plan and coordinate discharge planning, developing a care plan for post-hospitalization needs and ensuring continuity of care. The position is full-time, 40 hours weekly with a day shift and comprehensive benefits.

Qualified candidates will have a Master’s Degree in Social Work or a related mental health field and at least three years’ experience in a psychiatric health care setting, plus Crisis Prevention Certification within 30 days of

• Master’s degree in Social Work or a clinical related mental health field is preferred.
• Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in Case Management and discharge planning.
• Crisis Prevention Certification Trainings (Handle with Care) within 30 days of employment and prior to assisting with a restraining procedure.

Responsibilities

• Plan and coordinate activities related to discharge planning.
