Job summary

Qualifications

• High School Diploma or equivalent required; Bachelor's degree in Social Work, Counseling or related human service field preferred.
• One or more years previous related experience in a behavioral setting preferred.
• Previous experience with the client population served by the facility preferred.
• Previous experience in case management preferred.

Responsibilities

• Develops and implements activities and/or programs within scope of practice as per ISP.
• Collaborates with client and family to implement an effective service plan and explain options.
• Maintains comprehensive clinical records including assessments and progress toward recovery.
• Coordinates care with providers to ensure ISP implementation across services.
• Ensures discharge planning begins at admission and incorporates collateral input.
