Qualifications

• Case Management II certification is required or
• High school diploma or equivalent with 36 months of direct behavioral health experience
• 60 college credit hours with 12 months of behavioral health experience
• Bachelor's or Master's degree with 6 months behavioral health experience
• Bachelor's or Master's in Behavioral Health field preferred

Responsibilities

• Provide case management and rehabilitative services to individuals, families and groups.
• Provide crisis intervention services as directed by policy and protocol.
• Assess client needs and abilities and connect them with community resources.
• Complete all agency documentation of services thoroughly and timely.
• Assist clients to adapt to their environment and treatment.
• Maintain case management certification as required.
• Attend staff meetings and meet performance expectations.
• Travel as needed to transport clients or attend appointments.
