Job summary

Louisiana Blue in Louisiana is seeking a Behavioral Health Case Manager to coordinate and manage behavioral health services for members enrolled in Medicare and Commercial plans. You will conduct comprehensive assessments, develop care plans, coordinate services across inpatient, outpatient, and community settings, and monitor outcomes.

You will collaborate with internal teams, providers, families, and community resources, participate in crisis calls, and ensure regulatory compliance.

Qualifications

• Bachelor’s degree in nursing, healthcare, or related field OR master’s degree in social work, counseling, or behavioral health
• RN license in Louisiana can replace Bachelor's degree OR LPN license in Louisiana + 2 years of related experience can replace the bachelor’s degree
• Minimum of three years of experience in case management including behavioral health or crisis intervention
• LPC, LCSW, LMFT, RN, or LPN license required
• Strong clinical assessment and critical thinking skills
• Proficiency with electronic health records and case management systems

Responsibilities

• Conduct comprehensive behavioral health assessments to identify member needs, strengths, and goals.
• Develop, implement, and monitor individualized care plans with members and providers.
• Coordinate behavioral health services across inpatient, outpatient, and community settings.
• Facilitate level of care transitions and post-hospital follow-up.
• Provide education and support to members and families on conditions and resources.
• Participate in crisis calls with rapid assessment and emergency coordination as needed.
• Document all activities in compliance with regulatory standards.
• Engage in quality improvement and interdisciplinary case rounds.
