Job summary

CVS Health is seeking a Behavioral Health Case Manager / Member Navigator in Baton Rouge. The role supports members with behavioral health needs, guiding them through the healthcare system and keeping them connected to care.

Responsibilities include outreach after inpatient or ED events, arranging follow-ups, improving engagement with treatment plans, and addressing barriers to access, while coordinating with care teams. 40 hours weekly, full-time, with a comprehensive benefits package.

Qualifications

• Prior clinical and/or managed care experience in the healthcare industry or quality management.

Responsibilities

• Conduct outreach to members following behavioral health inpatient admissions and ED visits.
• Facilitate timely follow-up appointments.
• Improve member engagement with behavioral health treatment plans.
• Address barriers to accessing care.
• Connect members with community and social support resources.
• Coordinate with providers, care managers, and population health teams.
• Act on ADT notifications and discharge information to intervene quickly during high-risk transition periods.
• Support continuity of care and prevention of avoidable readmissions.
