Job summary

Louisiana Blue is seeking a Behavioral Health Case Manager to coordinate care for members enrolled in Medicare and Commercial plans. The role entails comprehensive assessment, care planning, service coordination across inpatient, outpatient, and community-based settings, and ongoing monitoring to ensure timely and effective care.

The ideal candidate holds an RN or LPN license in Louisiana (or a related master's in social work, counseling, or behavioral health) with at least three years of case

Qualifications

• Bachelor’s degree in nursing or related field; RN/LPN licensing in Louisiana required.
• Minimum three years of case management experience in behavioral health.
• Experience with Medicare/Commercial populations and managed care.
• Familiarity with CPT codes and billing practices.

Responsibilities

• Conduct comprehensive behavioral health assessments and identify member needs and goals.
• Develop, implement, and monitor individualized care plans in collaboration with members and providers.
• Coordinate behavioral health services across inpatient, outpatient, and community settings.
• Facilitate level of care transitions and ensure follow-up after hospitalizations or emergencies.
• Document all case management activities in accordance with regulations and standards.
