Job summary

COPE Community Services, Inc. is seeking a Behavioral Health Case Manager to develop, implement and update person-centered service plans for children and adolescents in behavioral health services. You will monitor progress, document outcomes, and coordinate care with primary care providers and other service providers.

The role emphasizes advocacy, collaboration, and timely communication of progress. It requires diverse qualifications paths and a commitment to safety, including background checks

Qualifications

  • Bachelor's or master's degree in behavioral health or related field.
  • Associate degree + 1 year behavioral health work experience.
  • HS diploma/GED + Peer Support Specialist cert + 2 years behavioral health experience.
  • HS diploma/GED + 3 years behavioral health experience.
  • Must obtain Level 1 Fingerprint Clearance Card.
  • Valid AZ driver's license with a 'clean' or 'acceptable' driving record.
  • Pass a pre-employment drug screen and background check.

Responsibilities

  • Develop, implement, and update person-centered individual service plans.
  • Monitor progress on plans, goals, objectives, and status; document progress.
  • Serve as a point of contact and advocate; communicate progress with providers and entities.
  • Coordinate all covered services (case management, outreach, crisis intervention, counseling).
  • Ensure collaboration with primary care providers and involved service providers.

Do you value professional growth, teamwork and respect?The Behavioral Health Case Manager (BHCM) develops, implements, and updates person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. Serve as a point of contact and advocate as needed, to ensure ongoing collaboration, including the communication of appropriate clinical information regarding progress of the person served with other individuals and/or entities with whom delivery and coordination of covered services is important (e.g., primary care providers, adult probation, other involved service providers).
