Job summary

COPE Community Services in Green Valley, AZ seeks a Behavioral Health Case Manager (BHCM). You will provide face-to-face, phone, virtual, and in-home services to support each member’s recovery journey and help develop person-centered service plans.

You will monitor progress on plans and collaborate with other providers, including primary care and probation, to coordinate covered services and track treatment goals in line with agency policies.

Qualifications

• Bachelor's or Master's degree required or equivalent.
• Associate degree plus 1 year behavioral health experience required.
• HS Diploma/GED plus Peer Support cert plus 2 years behavioral health experience required.
• HS Diploma/GED plus 3 years behavioral health experience required.

Responsibilities

Position Overview

The Behavioral Health Case Manager (BHCM) serves as a point of contact and advocate to members. The BHCM provides face‑to‑face, phone, virtual, and in‑home services as needed to support each member’s recovery journey. They develop, implement, and update person‑centered individual service plans, monitor progress on plans, goals, and objectives, and document progress on the service plan as defined by policies and procedures. The BHCM ensures ongoing collaboration and communication with other service providers, including primary care providers, adult probation, and other involved entities, to coordinate covered services and track treatment needs and goals.

Minimum Qualifications

• Bachelor's or master’s degree, no experience necessary
• Associate degree plus one (1) year behavioral health work experience
• High School Diploma or GED plus Peer Support Specialist certification plus two (2) years of behavioral health work experience
• High School Diploma or GED plus three (3) years of behavioral health work experience

Additional Requirements

• Must be 21 years of age and possess a valid AZ driver’s license with a clean or acceptable driving record.
• Driving record standards: no more than two moving violations, no more than one at‑fault accident, or any major violation (e.g., DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three years. A 36‑month driving record will be required at the time of job offer.
• Must be able to pass a pre‑employment drug screen and background check.
