COPE Community Services in Green Valley, AZ seeks a Behavioral Health Case Manager (BHCM). You will provide face-to-face, phone, virtual, and in-home services to support each member’s recovery journey and help develop person-centered service plans.
You will monitor progress on plans and collaborate with other providers, including primary care and probation, to coordinate covered services and track treatment goals in line with agency policies.
The Behavioral Health Case Manager (BHCM) serves as a point of contact and advocate to members. The BHCM provides face‑to‑face, phone, virtual, and in‑home services as needed to support each member’s recovery journey. They develop, implement, and update person‑centered individual service plans, monitor progress on plans, goals, and objectives, and document progress on the service plan as defined by policies and procedures. The BHCM ensures ongoing collaboration and communication with other service providers, including primary care providers, adult probation, and other involved entities, to coordinate covered services and track treatment needs and goals.
