Job summary

COPE Community Services, Inc. is seeking a Behavioral Health Case Manager to advocate for members and coordinate care across face-to-face, phone, virtual, and in-home modalities in Tucson.

The role emphasizes developing person-centered service plans and tracking progress within policies and procedures. Ideal candidates may hold a Bachelor's or Master's in a relevant field, or possess equivalent combinations of education and behavioral health experience, with strong communication and coordination

Qualifications

  • Bachelor's or master's degree required or Associate degree with 1 year BH experience.
  • HS Diploma/GED plus Peer Specialist cert plus 2 years BH experience.
  • HS Diploma/GED plus Medical Assistant cert plus 2 years BH experience.
  • HS Diploma/GED plus 3 years BH experience.

Responsibilities

  • Provide face-to-face, phone, virtual, and in-home services as needed.
  • Develop and update person-centered individual service plans.
  • Monitor progress and document service plan status.
  • Coordinate with primary care providers and other service providers.
  • Ensure collaboration and communication of progress with involved parties.

They provide face-to-face, phone, virtual, and in-home services as needed by each member's journey to recovery. They develop, implement, and update person-centered individual service plans (promoting member participation) in a timely manner. Monitor progress on plans, goals, objectives, and status on an ongoing basis with the person served, documenting the progress on the service plan as defined by policies and procedures. An unacceptable driving record includes (but is not limited to) the following:more than two [2] moving violations in the last three [3] years or;more than one [1] at-fault accident within the last three [3] years or;any major violation (i.e. DUI, Careless/Reckless Driving, Drag Racing, Hit and Run) within the last three [3] years.Other driving related violations may be considered unacceptable. At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.Applicants must be able to pass a pre-employment drug screen.This is a Safety Sensitive Position.
