Job summary

Arista Recovery is seeking a Case Manager to support the Clinical team in coordinating patient care throughout treatment. You will record communications, monitor activities, and help manage daily schedules in line with facility policies and state/federal standards.

The role requires proactive communication and collaboration with therapists to ensure clear discharge planning and community linkages. You will perform assessments, document progress, and assist with coordinating benefits and family

Qualifications

• Minimum 2 years of experience working with behavioral health needs (trauma, mental illness, substance use).
• Experience in case management within mental health or substance abuse settings preferred.
• Experience working on a multidisciplinary team.
• Knowledge of patient confidentiality laws and HIPAA requirements.

Responsibilities

• Supervise patients in daily activities within facility policies.
• Conduct initial case management assessments and develop care plans.
• Document daily progress notes and treatment team communications.
• Coordinate and document treatment schedules and activities.
• Facilitate communications between patients, families, and outside providers.
• Assist with discharge planning and instructions.
• Complete coordination of benefits documentation as needed.
• Maintain and document patient tours and schedules.
• Record and report any policy violations or safety concerns.
• Participate in trainings and apply new knowledge in practice.
