Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager to support children, adolescents and adults with a range of behavioral health needs. The role supports care planning, linkage to services, and crisis prevention in collaboration with licensed clinicians.

The position offers full or part-time hours, comprehensive training, and a benefits package while traveling within the service area as required.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Create service planning for individualized supports and treatment plans.
• Deliver linkage, interventions, and crisis prevention services to clients
• Serve individual clients or groups and maintain positive contacts with outside partners
• Assist in developing and implementing care plans and coordinating healthcare services

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• Bachelor's or Master's level degree in a related field or a high school diploma or three or more years of relevant field experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
