Job summary

ViaQuest Psychiatric & Behavioral Solutions in Ohio (Beachwood) is seeking a Behavioral Health Case Manager to support clients across age groups with behavioral health needs. The role involves coordinating services, delivering therapeutic supports in collaboration with licensed clinicians, and promoting daily living skills through crisis prevention and interventions.

You will work Monday–Friday, 9am–5pm with some flexibility for evenings or earlier starts.

Qualifications

• 18 years or older.
• High school diploma plus 3+ years in related field OR a Bachelor’s/Master’s in a related field.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician.
• Develop individualized service plans or care coordination.
• Provide linkage, interventions, and crisis prevention services.

$18-$21 based on education and experience plus monthly incentive program for exceeding productivity expectations

Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve.

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
