Who We Need

As a member of the Integrated Care Team, you will participate in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implement services to members in accordance with their plan. You will provide ongoing care and review progress, coordinating with all involved healthcare, behavioral health, and wellness providers while maintaining current documentation on members’ progress in managing chronic medical and psychiatric conditions, assisting individuals in achieving their recovery goals.

Minimum Qualifications:

  • High School degree required, and minimum four (4) years of related experience OR
  • Associates Degree in a related field with two (2) years of related experience OR
  • Bachelor's degree in a related field with one (1) year of related experience.
  • Case Managers are required to have the ability to transport members using their own vehicle. To meet this requirement, they must possess:

A valid Arizona (AZ) driver's license, a clean driving record, and current auto insurance and registration.

  • Must have a Level 1 fingerprint clearance card with no restrictions issued by Arizona Department of Public Safety OR be eligible to receive within eight (8) weeks of hire.
  • Bilingual (English/Spanish) preferred.

Job Summary (Including, but not limited to)

  • Works collaboratively with the integrated care team to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the Integrated Service Plan are implemented.
  • Develops, maintains, and manages the Integrated Service Plan (ISP) within the required time-frame.
  • Participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP).
  • Implements services to members in accordance with their plan and provides ongoing care and reviews progress.
  • Coordinates with all involved healthcare, behavioral health, and wellness providers and maintains current documentation on members’ progress in managing chronic medical and psychiatric conditions, assisting individuals in achieving their recovery goals.
  • To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and/or department.
