Position Overview: This role supports corporate objectives by utilizing demonstrated competency in Long Term Services and Supports (LTSS) to provide Case Management services. The position involves participating in case conferences with the Supervisor.

If you reside near Fremont and Custer Counties, CO, you will have the flexibility to work remotely* as you take on challenging cases.

Primary Responsibilities:
  1. Conduct intake, screening, referral, assessment/reassessment, develop support plans, provide ongoing case management, monitor members' health and welfare, document contacts and activities, develop resources, and close cases.
  2. Maintain confidentiality of patient information as per the UHG Employee Handbook and all legal requirements.
  3. Perform monitoring contacts as prescribed, at least quarterly or more frequently based on the member's condition.
  4. Conduct face-to-face contacts at least every six months, updating assessments and plans as needed.
  5. Complete reassessments annually or more frequently if warranted, and additional assessments as required.
  6. Monitor the delivery of services and supports as outlined in the Support Plan and Prior Authorization Requests.
  7. Attend training sessions provided by the Department and agency in-service and staff development programs.
  8. Ensure all work complies with the CMA Contract Agreement, including accuracy, timeliness, and completeness.
  9. Immediately refer suspected cases of mistreatment, abuse, neglect, or exploitation to appropriate protective services or law enforcement.
  10. Take on special assignments and projects as needed, including system assessments and development.
  11. Travel is required; must have reliable transportation and meet licensure and insurance requirements if driving.
  12. This position addresses efforts focused on medical, behavioral, and social health determinants to improve community well-being.

Required Qualifications:
  • Either a Bachelor's degree in human services, nursing, social work, psychology, or public health, OR 5+ years of relevant LTSS experience.
  • At least 1 year of experience with MS Office applications.
  • Reliable transportation and ability to travel up to 25-30% within the territory.
  • Reside near Fremont/Custer Counties.
Preferred Qualifications:
  • Experience as a caseworker or case manager with LTSS populations, in either private or public agencies, may substitute for education on a year-for-year basis.
  • Physical ability to move between office and field locations, navigate terrains, and transport equipment.

The hourly rate ranges from $23.70 to $46.35, based on full-time employment, location, experience, and other factors. Benefits include comprehensive packages, incentive programs, stock purchase options, and 401k contributions. Benefits are subject to eligibility.
