Job summary

accesscommunity is seeking a Case Worker – Behavioral Health to perform intake, assessments, counseling, and case management in a behavioral health setting. The role involves connecting clients with services, documenting progress, and ensuring compliance with funding guidelines and policies.

The position requires a bachelor’s degree in social work or related field, 2–3 years of relevant experience, and QMHP credentials.

Qualifications

Bachelor’s degree in Social Work, Psychology, or related field.
Minimum 2–3 years of related experience (based on functional area).
QMHP adults/children credentials required within specified timeframes.

Responsibilities

Identify in‑need individuals and conduct intake and crisis intervention; complete forms.
Connect clients to services, support, and resources.
Research and locate available help outlets, in-house or external referrals.
Explain processes, rules and regulations surrounding eligibility and benefits.
Engage clients in developing individualized goals and plans.
Advocate on behalf of clients with other agencies/companies.
Follow up on services to ensure participation and compliance.
Document services per agency QA plan and funding guidelines.
Report on program progress and participate in reviews and trainings.
Maintain confidentiality of clients and agency information.
